「評価支援シート」とは
・ロジックモデルに指標を設定し、その指標の実測値をロジックモデルの上に配置したものを「評価支援シート」（ないしは「評価シート」）と呼んでいます。
・ロジックモデルを作っていない地域でもロジックモデル案を想定できます。
・指標を十分に設定していないところでも指標を設定したかたちが分かります。
・データを計測していない地域でもデータを得ることができます。
・ロジックモデルと指標を設定し、そのデータを計測して準備してから年次進捗評価や中間評価の作業に入りますが、そうした作業ができていない地域でも疑似的にその段階まで至れます。
・また、地域のだれでもすぐにそのデータが得られます。
・ただし、オープンデータから構成しているので、厚労省データブックや都道府県独自計測データで公表されていないものについてはデータがNAとなります。

What is an "Evaluation Support Sheet"?
☑ An "Evaluation Support Sheet" (or "Evaluation Sheet") refers to a tool where indicators are set within a logic model, and the actual measured values of those indicators are placed on the logic model.
☑ Even in regions that have not created a logic model, a draft logic model can be assumed.
☑ In areas where indicators have not been fully established, the sheet allows visualization of how those indicators could be set.
☑ For regions that have not yet measured data, the sheet provides access to relevant data.
☑ Typically, regions set up a logic model and indicators, measure the data, and prepare for tasks such as annual progress evaluations or midterm evaluations. However, even regions that have not reached that stage can simulate the process up to that point using this sheet.
☑ Moreover, anyone in the region can easily access this data immediately.
☑ However, since the sheet is composed of open data, any data not published in the Ministry of Health, Labour and Welfare data books or in independently measured prefectural data will be marked as "NA" (not available).
